Let’s be honest, no one likes filling out forms. But before you can place an order, you’ll need to complete our registration process and be verified by our team. To keep it simple, we’ve broken it down for you into three easy steps:

Step 1

Fill out your Application for Medical Marijuana

You’ll need to do this bit yourself. Then, either you or your doctor/clinic can submit the completed form to us by email, mail or fax. You’ll find a downloadable pdf form at the bottom of this page.

Step 2

Ask your doctor to complete the Medical Document form

Think of this as your prescription - without it we can’t fulfil your script. We accept completed forms by mail or fax (as long as it comes from your doctor’s official fax number). You can use our form below or your doctor’s own template, provided it includes all the info required for a cannabis prescription.

Step 3

Check your inbox for your approval email

If you don’t hear back from us straight away, don’t worry! We do our best to get you verified as fast as we can, but it can sometimes take a few days to complete the process. Once you’re approved, that’s all the admin stuff over with: you’re ready to start ordering.
